Campout Medical Form

Please download/upload your medical forms for Pack 48 campouts and events here.  We can access them for one year from submission for all campouts/events which require a medical form.  Once you download the form, please complete it for each camper who will attend an event.

Troubles Getting the form?

• https://www.scouting.org/health-and-safety/ahmr/ contains the medical forms for download. 

• If you can't download the form, please send an email to info@goldenpack48.org and we will send you the form.

Troubles Submitting?

• You must be signed into a google account to upload the form. 

• If you do not have a google account, please send an email to info@goldenpack48.org with the form attached.
